- ShopWrk forms are the ones you build here: quote requests, consultation bookings, anything you want on your website or shared as a link.
- The booking form is a separate, purpose-built form behind your booking page. See Calendar and booking.
- Facebook Lead Ads forms live in Meta’s system. ShopWrk imports their submissions so those leads land in the same place as everything else.
Building a form

Start a new form
Click New form. Give it an internal name for your team and a customer-facing name that shows on the form itself.

Add steps and fields
Build the form step by step. Each step holds one screen of fields; reorder them until the flow feels natural. Mark fields required when an answer changes what you do next, like vehicle year or service interest.

Use custom fields where you need them
Fields can bind to your custom contact fields, so submissions land as structured data on the contact instead of buried in notes.

Publish
Set the public link slug, then switch the form from draft to published. Preview before you share.

Share it
Copy the public link for bios, DMs, and email signatures, or embed it in your website. The link keeps working while the form stays published.
Importing Facebook Lead Ads forms
If you run Lead Ads on Facebook or Instagram, connect Facebook Lead Ads first (see Instagram, Messenger, and Facebook Lead Ads), then open Marketing → Forms and import the lead form. Map each question to a ShopWrk contact field once; after that, submissions flow in automatically with the mapping applied.Working submissions
Every submission, from every source, collects in the Forms area:- Review. Open a submission to see exactly what the customer entered. Mark it reviewed so your team knows who has handled it.
- Convert. Submissions create contacts automatically; add vehicles, estimates, or jobs from the contact like any other lead.
- Export. Pull submissions to CSV for analysis or handoff.
- Delete. Remove junk or test submissions permanently.
Automating follow-up
Two triggers matter here: ShopWrk Form Submitted fires for ShopWrk forms, and Meta Lead Form fires for imported Lead Ads. A common setup texts new leads within a minute and assigns the lead to whoever owns that service line. See Automations and the lead closer for how far you can take it.Form states and fixes
Unpublishing (archiving) keeps old submissions but stops new ones, which is what you want for expired promotions. A form nobody finds usually needs its link somewhere real: website, bio, email signature.
Form access follows team permissions. Members without marketing access don’t see Forms in the sidebar at all.
